There are important details for medications, skincare products & medical services/procedures that need to be avoided before we execute permanent makeup on the skin.
Reviewing this document beforehand will ensure there are no surprises for myself or the client. Most importantly, adhering to the pre-care guidelines will optimize long term results and enhance client satisfaction
All booked clients must complete this Release & Consent form prior to a service.
It's ideal to have this completed 24+ hours before the time of your appointment
This is a legally binding document and does provide consent that you reviewed the above Pre-Care Instructions & Contraindications in full.
Aftercare instructions are critical to be adhered to in order to get the ideal results.
It is recommended to review this before booking in order to understand limitations and also be able to ask any questions at the time of your appointment.
All clients can receive a printed copy of this at the time of their appointment if desired.
